What happens after you make an appointment?

Check Your Email

Once you have booked your appointment with a nurse practitioner, you will be sent all of the required medical questionnaires and assessment forms to your email.

Please make sure to complete all forms at least 48 hours prior to your appointment.

Medical Questionnaires & Assessment Forms (30 - 45 minutes)

In your email, you should see five questionnaire/assessment forms. These forms are designed to give the nurse practitioner a clear picture of your overall health and symptoms. The assessment forms will include the following:

  • Wender Utah Rating Scale
  • Adult ADHD Self-Report Scale
  • Patient Health Questionnaire (PHQ-9)
  • Generalized Anxiety Disorder (GAD-7)
  • Mood Disorder Questionaire

Take your time in filling out these forms, and make sure to submit them when you’re finished.

3.

Assessment Appointment with Nurse Practitioner (60 minutes)

In your first virtual meeting, you and the nurse practitioner will go over the medical questionnaires and assessment forms together.

During this stage, the nurse will ask you for any further information or clarification needed regarding your medical history and symptoms.

The nurse will then ask you to book a follow-up appointment for 2 weeks from the date.

Diagnostic Appointment & Treatment Plan (60 minutes)

During this appointment, the nurse will review her findings and inform you if you do or do not meet the criteria for an ADHD diagnosis. 

Regardless of diagnosis, you will be provided with a treatment plan. What is included in the treatment plan will vary from person to person. A treatment plan could involve suggestions for types of therapy, medical treatments, and other helpful resources.

5.

Follow Up Appointments and On-Going Care

If you would like to continue working with the nurse practitioner, you will be given the option to book follow-up appointments ($70 per session). 

During these follow-up sessions, you can work with the nurse to discuss your progress and make adjustments to your treatment plan where necessary.

Frequently Asked Questions

How long does the assessment take?

The online assessment typically takes about 30-45 minutes to complete, and the video calls with your nurse practitioner are 60 minutes. 

How do I schedule my appointments?

When you go to pay for your assessment, you will be prompted to book your first call with one of our nurse practitioners. After your payment is complete, your appointment is confirmed.

Am I able to continue working with my nurse practitioner after the assessment?

Yes, we offer ongoing support at an additional fee ($70 per session) through follow-up appointments where you can work with the nurse practitioner for further support and to make adjustments to your treatment plan as necessary.

What happens if I am not diagnosed with ADHD?

If your assessment finds that you do not meet the criteria for having ADHD, you will still receive an effective treatment plan. Our nurse practitioner will recommend any therapies or other assessments that might be useful for you.
